When filling out this form, please be as flexible as possible. Due to the volume of requests, Mayor Deegan may not be able to meet with you. The City of Jacksonville also offers other avenues of response including 630-CITY to assist with citizen concerns.
The more specific the information you provide, the better we'll be able to accommodate your request. Please submit only one form per organization to ensure effective communication.
