Event Request Form
Please fill out this form completely and with as much detail as possible. All requests are reviewed and managed exclusively by the Mayor's Executive assistant. While we will make every effort to accommodate your request, please note that the mayor’s schedule is subject to change.
You should see a confirmation message pop up upon successful submission of your request. If this does not happen, please review the form to ensure all information was input properly. You will also receive a confirmation email to the email address you provided, shortly after submitting your request. If you do not receive an email confirmation, the request has not been received by our office and you will need to submit the request again.
