United Way 2-1-1

ElderSource

ElderSource is a private, not-for-profit agency designated by the state as the Area Agency on Aging and Aging & Disability Resource Center (ADRC) for Northeast Florida. ElderSource serves as a focal point where older adults, adults with disabilities, caregivers, and the general public can receive information, referrals, assistance, and answers on aging-related issues.

Aging True

Formerly Cathedral Foundation of Jacksonville, Inc. and Urban Jacksonville, Inc., Aging True provides a variety of programs for seniors, caregivers, and families in Jacksonville and Northeast Florida.

Programs may include: Nutrition, care coordination, home health services, personal and in-home care, Meals on Wheels, caregiver support, Community Care for the Elderly, Alzheimer’s Disease Initiative, emergency alert response, counseling services, and more.

Aging True’s professional staff works with seniors and their families to select the most appropriate program or combination of services, including health, housing, financial, and nutritional education and other vital areas of assistance.
