Request for Change of Address
If you need to have your address changed you will need to complete and submit an Address Change Application. Please note that there are two versions of the application. One application is for the property owner and one application for the tenant. If you are not the property owner, an Affidavit of Property Ownership & Designation of Agent form will need to be completed and provided with the Address Change Application. Once the application is completed, signed and notarized please email or mail to the Addressing office at:

NOTE: The Public Works Department will only change an address when it is deemed necessary in order to ensure that a property can be found quickly in an emergency. A request for a change of address will be reviewed to determine if the change is necessary and consistent with the City of Jacksonville Addressing and Street Naming Policy.
