Private Provider

This page provides the requirements and procedures for Private Providers working on projects in The City of Jacksonville. It is intended to help ensure compliance with Section 553.791, Florida Statutes, and to outline the documentation, inspection, and plan review requirements necessary for permit processing.


Responsibilities of the Private Provider

Private Providers are responsible for:

  • Registering with the City of Jacksonville, as required.
  • Submitting all required Private Provider documents.
  • Performing plan reviews and inspections in accordance with Section 553.791, Florida Statutes, when contracted to do so.
  • Completing scheduled required inspections through your JaxEPICS profile.
  • Coordinating with the City of Jacksonville throughout the permitting process.
  • Requesting the Certificate of Occupancy or Certificate of Completion after all required inspections and documentation have been completed and submitted.


Registration of Private Provider

User Profile

Private Provider firms must have a company user profile in JaxEPICS to schedule and result inspections for permits in the City of Jacksonville.

Important:

  • The Profile must be created for the Private Provider firm, not an individual employee.
  • The firm's Qualifier's professional license must be linked to the account.

Create a User Profile

  1.  Complete the Registration form in the link below.
  2.  When Registration form is complete, submit it for approval.

Documents required for registration

  1. Certificate of Insurance (Professional Liability)
  2. Florida License Verification (DBPR)
  3. Registration form 
  4. Email addresses for the Qualifier 
  5. Email addresses for licensed Plan Reviewers or Inspectors


Licensed Professional

The firm's Qualifier’s must have their professional licenses linked to the company user profile.

Required Permit Documents

  •  Notice to Building Official (NTBO)
  •  Fee Owner Authorization
  •  Plan Compliance Affidavit (PCA - if applicable)
  •  Property Owner Authorization Form

 

Requirements for Private Provider Plan Review

  • Registering for a Plan Review Profile 

If the Private Provider will be submitting permits for Private Provider Plan Review, in conjunction with their “Private Provider” profile they will also need a JaxEPICS “Business” profile as a Design Professional. That Registration form is available in the link below and should be returned to BIDRegistrations@coj.net to be processed. 

 

Scheduling and Resulting Inspections

Scheduling Inspections

The Fee Owner or Fee Owners contractor is responsible for scheduling all required inspections through the JaxEPICS portal. Private Provider Firms added as a resource to the permit may have access to schedule inspections.

Resulting Inspections

Inspection results must be entered into JaxEPICS. Access to the portal is provided by City of Jacksonville Private Provider Supervisor after the Private Provider firm has been registered.

The portal allows Private Providers to:

  • View scheduled inspections.
  • Transfer scheduled inspection to other licensed resources in your company profile.
  • Result inspections assigned to licensed resources in company profile. 
  • Enter inspection comments into permit.
  • View previously submitted inspection results and comments.

Inspections are to be resulted in JaxEPICS, the following fields are required:

  • Inspection Date (MM/DD/YYYY)
  • Inspection Status

Accepted inspection statuses are:

  • Passed
  • Passed with Comments
  • Partial Passed
  • Failed


Certificate of Compliance

After all required inspections have been completed, the Private Provider must list all completed inspections resulted by the Private Provider on the Inspections Certified by Private Provider form (state form to replace once issued):

  • All Inspections Certified by Private Provider forms should be sent to BidPrivateProviders@coj.net to be processed. 
  • Any supporting documentation used to approve an inspection (e.g., Engineer's Letter).

Before the Certificate of Compliance can be accepted:

  • All required inspections must be resulted.
  • All associated permits must be satisfied.
  • All applicable fees must be paid.


Certificate of Compliance Verification 

The City of Jacksonville will verify that the Certificate of Compliance and all supporting documentation are completed.

Once checked for completeness is approved, the Private Provider Agency Review with be approved and the permit will move from active to finalized. The Certificate of Occupancy or Certificate of Completion will then be automatically issued and available in Printable Documents on the permit tab.
